About the Role As a Nurse Practitioner in Primary Care, you will deliver comprehensive, patient-centred care across assessment, diagnosis, treatment, and ongoing management of acute and chronic conditions. You will play a key role in improving access to healthcare and supporting continuity of care within the local community.
